If you are a relative calling on behalf of a family member, privacy regulations require that we have written or verbal permission from the member or beneficiary to discuss their personal information. Verbal permission can be provided by phone and is good for 14 days.

For the Appointment of Representative Form: Written permission is good for one calendar year.

For the Personal Representative Form: Written permission is valid until you notify us of any changes.
